Resolution:
The plastic pouch Square D offers is 8003115901 and the standard 42 circuit directory card is 8003115801.  Note that the directory card is printed on one side with circuits numbered odd/even, that is, the left column of breakers number 1-41 odd, and the right column numbered 2-42, even.  On the other side, the directory card is numbered sequentially, with the left column of circuits numbered 1-21 and the right column 22-42.
