Which QO and HOM AFI breakers are affected by the 2004 recall?

What are the date codes for the QO-AFI and HOM-AFI breakers that are affected by the 2004 recall?

Product line:
Miniature arc-fault circuit breakers

Resolution:
This product recall includes all catalog numbers for QO, HOM, and QOB AFI circuit breakers with blue test buttons manufactured between March 1, 2004 and September 23, 2004.

Identification of circuit breaker
Catalog numbers affected include the following:
  • QO115AFI, QOB115AFI, HOM115AFI
  • QO120AFI, QOB120AFI, HOM120AFI
  • QO115VHAFI, QOB115VHAFI
  • QO120VHAFI, QOB120VHAFI
Date Codes include the following:
  • CN, DN, EN, FN, GN, HN, and JN
The recall is now being handled by Square D/Schneider Electric Field Services in Charlotte, NC at 1-800-634-8730.  However, return the product to your local distributor.


DETAILS
DISTRIBUTORS, CONTRACTORS, HOMEOWNERS
This recall is only for Blue Push Button AFI QO and HOM breakers in a specific date range CN, DN ,EN, FN, GN, HN, JN (see attached afi brochure.pdf for complete details).
  • If the push button is a different color, then this recall does not apply.
  • If the the date code is another combination of letters, then this recall does not apply.
The date code is indicated by a 2-letter stamp, in RED letters, located on the white sticker "made in Mexico" at the end of the breaker where the black circuit wire is connected to the breaker. In order to see this sticker, the breaker must be removed from the panel by a qualified electrician. If the date code is CN, DN ,EN, FN, GN, HN, or JN, then it is part of this recall. If the date code is another combination of letters, then the breaker is not under recall. If the Date Code is not legible, we will assume it is affected and it should be replaced.
Distributors are asked to follow the standard procedures for returning product, i.e. contact your Customer Service Representative at 1-888-SQUARED (1-888-778-2733) to arrange for a return authorization. Box stores can also contact this number if management is not aware of the recall. HOMEOWNERS
First read the above paragraph. After reading it, if your home has blue push button AFCI circuit breakers, your next step is to contact an electrician that is familiar with the Square D recall. The electrician should not charge you for material, since they can take the affected breakers, or any breaker with a Blue push button for which you cannot read the RED LETTER Date Code, to a Square D/Schneider Electric Distributor or Retail Box Store (Home Depot or Lowes).  They will swap the breakers, and there is no cost to you for the new breaker. The Electrician should only charge you for his labor. The Distributor/Retail Box store can return the affected  breakers to square D /Schneider Electric for credit.

  • Before hiring an electrician be sure that they confirm to you that they will only invoice you for labor!!.
  • If the breakers do not need to be changed, then we (Schneider Electric) will still reimburse for the labor to inspect Blue Push button AFI QO and HOM breakers
Homeowners, for reimbursement please submit the following
  1. Attachment 2 in the file AFI Breaker - Homeowner information.pdf, OR a copy of receipt on the exchange of breakers.
  2. A copy of  Electricians invoice for labor charges, to inspect and change out the breakers if necessary. Note:  If the breakers do not need to be changed then we will still pay for the labor to inspect.
  3. Download and fill out W9 Form (please see attached). This W9 is needed by accounting to set you up as a temporary customer in our system in order to issue you a reimbursement check.
